Comparison of Cold Kit and Cassette-Based Automated Synthesis Methods for 68Ga-PSMA-11

Abstract
Purpose: This study compared a cold kit protocol and a cassette-based automated system for routine synthesis of 68Ga-PSMA-11. Materials and Methods: We retrospectively analyzed 70 cold kit batches and 191 cassette-based batches. Synthesis time, decaycorrected radiochemical yield (RCY), coefficient of variation (CV), pH, radiochemical purity (RCP), sterility, and visual inspection were evaluated. Results: The cold kit method showed shorter synthesis time and higher RCY with lower CV than the cassettebased method. However, both methods satisfied pharmacopoeial acceptance criteria for RCP, pH, sterility, bacterial endotoxin and visual appearance in all batches. Conclusion: Cold kit and cassette-based synthesis provide 68Ga-PSMA-11 of acceptable quality, with different profiles in synthesis efficiency and operational characteristics. The choice of method should be based on the clinical environment and institutional resources.
